Young man has rare disease, never recorded in Vietnam

The 19-year-old male patient has a rare disease that has never been recorded in Vietnamese medical literature. Many months ago, the patient suffered from mouth ulcers and scald lesions scattered all over his body

On June 27, Bach Mai Hospital (Hanoi) reported a rare case that doctors cured after 7 months.

Previously, in November 2023, a 19-year-old man in Thanh Hoa had erosive ulcers on the oral mucosa and purple-red patches and scalds scattered all over his body. At the hospital for examination, the doctor discovered a large tumor behind the peritoneum.

The patient was transferred to the Central Dermatology Hospital and admitted to Bach Mai Hospital in mid-December 2023. Based on the skin biopsy results, the doctor diagnosed the patient with toxic epidermal necrolysis.

In early 2024, the patient was diagnosed with paraneoplastic Pempigus, accompanied by bilateral ureteral stones, physical exhaustion, and erosive skin lesions covering nearly 70% of the body; have inflammatory myofibroblastoma.

According to doctors, Pemphigus is a rare disease, not yet found in Vietnamese medical literature. In the world, the rate of people with the disease is 1/1 million people.

Although surgery was indicated, due to his weak health, the patient was discharged from the hospital for nutritional supplementation. However, 3 days later, the disease relapsed.

Last April, the patient had new outbreaks with symptoms of fever, widespread progressive skin erosion, and ulceration of the oral mucosa. Patients must undergo plasma exchange, nutritional treatment, and antibiotics. At the end of May, realizing that the patient’s condition was suitable, the doctor performed surgery to remove the tumor. After nearly 1 month of treatment, the doctor re-evaluated the patient’s overall condition and discharged the patient.

At the end of the treatment process, the male patient was paid nearly 700 million VND by health insurance, and his family co-paid more than 53 million VND.

According to doctors, paraneoplastic Pemphigus is a rare disease believed to be related to lymphoproliferative disorders.

In a statistic reviewed by the US Food and Drug Administration of over 100,000 patients with non-Hodgkin’s lymphoma and chronic lymphocytic leukemia, 12 cases were recorded with features of paraneoplastic pemphigus.
